Palmetto Regional Health System (PRHS) sought greater visibility into emergency department performance and patient experience metrics to reduce wait times, improve patient satisfaction, and optimize resource utilization. Leadership needed an interactive solution to monitor key indicators, identify trends, and support data-driven decisions that enhance care delivery and operational efficiency.

Technical Resources

This project was developed using Microsoft SQL Server, Tableau, and Excel. SQL was used for data preparation, validation, KPI calculations, and dashboard-ready reporting logic, while Tableau supported interactive Emergency Department and patient experience analysis. An end-to-end healthcare analytics solution was designed using Excel-generated sample data, Microsoft SQL Server, and Tableau. Operational data was modeled, validated with SQL, and transformed into executive dashboards for KPI monitoring, trend analysis, and executive decision support.

The solution provides leadership with a centralized view of Emergency Department patient flow, staffing efficiency, throughput, operational trends, and patient experience metrics.

Morning arrival surges increased provider demand and contributed to longer door-to-provider times.

Higher patient volumes aligned with increased Left Without Being Seen (LWBS) rates, highlighting opportunities to improve staffing alignment and patient flow.

These findings indicate that demand patterns, staffing alignment, triage workflow, and patient throughput should be evaluated together when addressing Emergency Department performance.

DEFINE

The project began by partnering with executive leadership to define the primary operational challenges affecting Emergency Department performance. Key concerns included increasing patient volumes, prolonged door-to-provider times, inconsistent patient throughput, elevated Left Without Being Seen (LWBS) rates, and opportunities to improve the overall patient experience. Project objectives, success metrics, executive stakeholders, and KPIs were established to align the analysis with organizational priorities.

MEASURE

A comprehensive operational dataset was developed and validated using Microsoft SQL Server to establish a reliable performance baseline. Metrics including patient volume, arrival patterns, ESI, door-to-provider time, length of stay, provider utilization, patient disposition, LWBS rate, and patient satisfaction were measured.

ANALYZE

SQL queries and interactive Tableau dashboards were used to identify operational trends, bottlenecks, and root causes. Analysis revealed that peak morning arrivals increased provider demand, resulting in longer wait times and higher LWBS rates, highlighting opportunities to better align staffing with demand.

IMPROVE

Recommendations included optimizing triage workflows, aligning staffing schedules with peak arrival periods, improving patient throughput, implementing executive dashboards, and standardizing operational performance reviews. Initiatives were prioritized based on expected impact.

CONTROL

An executive performance monitoring framework was developed using KPI scorecards, dashboards, and routine operational reviews to evaluate improvement initiatives, identify new optimization opportunities, and sustain long-term performance improvements.

Successful implementation of these recommendations is expected to improve Emergency Department throughput, reduce patient wait times, optimize provider utilization, decrease LWBS rates, enhance patient satisfaction, and strengthen executive decision-making through continuous KPI monitoring.

Aligning staffing and triage workflows with demand patterns can improve patient flow and reduce operational bottlenecks, while standardized performance reviews provide leadership with a consistent mechanism for monitoring progress.

This project demonstrates an end-to-end healthcare analytics and performance improvement methodology—from defining the business challenge and developing a relational SQL database to creating executive dashboards, identifying root causes, developing strategic recommendations, designing an implementation roadmap, and establishing ongoing performance monitoring.

The project demonstrates how operational healthcare data can be translated into actionable insights that support better Emergency Department performance, patient flow, and patient-centered care.
